Stephanie Duininck, FNP-C, L.Ac

Stephanie Duininck is a Board-Certified Family Nurse Practitioner and Licensed Acupuncturist with degrees from the University of Denver, Southwest Acupuncture College, and The George Washington University. She brings six years of acupuncture experience to the Center for Lifetime Health and is excited to offer integrated services in family medicine and Traditional Chinese Medicine.

Passionate about patient-centric care, Stephanie aims to empower her patients to be actively involved in their own healthcare. She seamlessly blends nursing and Traditional Chinese Medicine in her holistic approach to treatment.

Dr. Jon Hlavinka Headshot

Jon Hlavinka, MD

Meet Dr. Hlavinka, a seasoned family physician dedicated to providing top-quality care to patients of all ages. His journey into medicine was inspired by his roots in rural North Dakota and emulating his father's career as a doctor. Dr. Hlavinka acquired his medical degree from the University of North Dakota, followed by a residency at the University of Washington in Spokane. He further enriched his professional journey by serving as a medical director for a Native Association in Kodiak, Alaska, catering to healthcare needs in remote areas for six years.

Subsequently settling in Idaho, Dr. Hlavinka has significantly contributed to family medicine for many years. His diverse medical experiences include serving as a high school team doctor and a nursing home director and providing comprehensive care across life stages. His recent practice focuses on adolescent and adult medicine.

Dr. Hlavinka's interests extend beyond medicine. He enjoys gardening and maintaining a hobby farm inhabited by various animals. An outdoor enthusiast, he loves to spend his leisure time in the mountains, hiking, snowshoeing, paddling, and cycling.

Julia Jensen, DO

Hailing from a rural area outside Boyertown, Pennsylvania, Dr. Julia Jensen is a dedicated family physician with a rich academic and professional background. She studied at Drexel University and the Philadelphia College of Osteopathic Medicine, earning a DO degree (Doctor of Osteopathic Medicine). She further honed her medical skills during her family medicine residency at CHRISTUS Santa Rosa in San Antonio, Texas, and has been practicing family medicine since 1999. In 2020, she achieved diplomate status from the American Board of Lifestyle Medicine.

A staunch believer in holistic treatment, Dr. Jensen considers all aspects of a person's health and wellness. To bolster this approach, she was certified in Lifestyle Medicine in 2020, enhancing her ability to guide patients toward lifestyle changes as primary therapy in managing their diseases and overall health.

Her battle with breast cancer in 2017 underscored the significance of maintaining healthier lifestyle habits. She incorporated a predominantly whole-food, plant-based diet, meditation, running, and biking into her regimen, which proved crucial in her journey to survivorship. Guiding patients through their health journey's ups and downs and helping them achieve and maintain lifelong health has become her true calling.

Dr. Jensen is an active outdoor enthusiast. She enjoys running on trails, mountain biking, practicing yoga, hiking, and camping with her husband and three children during her leisure time. Since moving to Boise in 2013, she has found a joyful and fulfilling home.

Terryn Martin

Terryn Martin, DNP, APRN, FNP-C

Terryn Martin, a Board Certified Family Nurse Practitioner, is a passionate healthcare professional with an eclectic background. She earned her Doctorate Nurse Practitioner Degree in Family and Individual Care from Montana State University in May 2017. Before stepping into her role as a Nurse Practitioner, Terryn amassed over nine years of experience as a registered nurse, primarily caring for neonatal intensive care, newborn nursery, and maternity patients. She also dedicated time to the academic side of nursing, serving as a nursing instructor and mentor.

Terryn's passion for family practice is all-encompassing, with a particular fondness for caring for pediatric, adolescent, and GYN patients and those managing diabetes and other chronic illnesses. Her approach to wellness is comprehensive, considering physical, emotional, and spiritual aspects. Terryn's foremost desire is to establish enduring relationships with her patients, empowering them with the tools to prevent illness, manage disease, and maximize their health.

Family is central to Terryn's life. She enjoys the company of her devoted husband, energetic daughter, and tender-hearted son. An outdoor enthusiast, she relishes exploring nature with her family through activities like backpacking, hiking, fly fishing, skiing, and water sports. She enjoys indulging in homely pursuits such as quilting and crafting on quieter days.

Colleen Shackelford, DNP, APRN, FNP-C

Colleen Shackelford is a Board Certified Nurse Practitioner with a notable academic background. She earned her doctorate (DNP) from Gonzaga University, following her studies at the University of Portland (BSN, RN) and The George Washington University (MSN, FNP). She maintains active memberships with the Nurse Practitioners of Idaho, The American Academy of Nurse Practitioners, and the Sigma Theta Tau International Honors Society of Nursing.

Colleen brings over 10 years of diverse practice experience, offering care to Idahoans of all ages. She finds joy in caring for families, often serving several generations within the same family. Her healthcare approach strongly emphasizes lifestyle modifications as part of disease management and prevention, supported by effective health promotion strategies.

A strong advocate for education, Colleen contributes to the growth of the future NP workforce. She has dedicated over 1,000 hours of clinical preceptorship to numerous NP and PA students and has served as adjunct faculty for Idaho State University's NP program.

Honored with the NP State Award for Excellence in 2019, Colleen is recognized for her work in NP Policy. She actively collaborates with the Nurse Practitioners of Idaho, working diligently to reduce barriers to NP practice and promote a healthier Idaho through her role as Legislative Committee Chair and President-Elect. Additionally, she shares her knowledge and experience as a speaker at state and national healthcare and health policy conferences.

Beyond her professional endeavors, Colleen is an outdoor enthusiast. She relishes running, biking, skiing, and traveling, and particularly enjoys camping and exploring Idaho's beautiful outdoors with her husband and two sons.

Michelle B. Carter

Michelle B. Carter, MED, RDN, LD

Combining her passions for education, movement, and exquisite food, Michelle has been shaping her career as a registered dietitian since 1995. She graduated Magna cum Laude with a Bachelor of Science in Human Nutrition from the University of Houston while simultaneously pursuing professional dancing with the Delia Stewart Dance Company. Her academic journey continued with a dietetic internship at Baylor College of Medicine in Dallas. She was handpicked to work at the Cooper Clinic in Dallas, where a team of dietitians, exercise physiologists, and board-certified physicians focused on comprehensive preventive exams. This experience sparked Michelle's interest in disease prevention and led her to earn a Master's in Exercise Physiology from the University of Houston.

After completing her studies, Michelle was chosen to set up the nutrition department at the esteemed Houstonian Health Club and Spa. Her diverse roles have included:

  • Serving as the sports dietitian for Team Houston.

  • Appearing weekly on Channel 13's Health Beat in Houston.

  • Working as the Boise Ironman Sports Nutrition Coach for Performance High in Boise, Idaho.

  • Michelle is a gifted speaker and media expert and has been an Idaho nutrition spokesperson for the Academy of Nutrition and Dietetics.

Michelle's expertise spans difficult weight loss, inflammation combat, autoimmune disease management, corporate wellness, and disease prevention. After being diagnosed with Hashimoto's, an autoimmune thyroid disease 2013, Michelle understands the health challenges even the healthiest individuals can face. She strives to empower her clients by demonstrating how minor dietary adjustments can significantly reduce disease risks and enhance well-being.

An active Academy of Nutrition and Dietetics member, Michelle is part of dietetic practice groups focused on Sports and Human Performance Nutrition, Cardiovascular Health and Well-being, and Diabetes. Guided by her motto "eat well, perform well," she educates her clients on practical, flavorful ways to boost their diets with nutritious foods.

Outside of her professional life, Michelle enjoys hiking with friends, weight training, cooking dinner with her husband, exploring Antique World Mall, laughing, and a good comedy. She is a proud parent to two children who relish everything Idaho offers.
