What insurance plans do you accept?

We accept most individual and commercial healthcare plans. We are not contracted with Medicare and Medicaid. If you are a self-pay patient, we offer a 20% discount the day of service.

How do I access my patient portal?

Your Patient Portal can be accessed through the Healow application either on the web or through the mobile app. Go to ‘My Chart’ on our website, to access the web version. If downloading the app, our office code is DJFAAA.

I cannot login into my patient portal, what do I do?

            Please call our office in order for us to reset your Patient Portal credentials.

When will I know my lab results?

Lab results will be posted to your Patient Portal once your provider has reviewed them. You should receive a call within seven days about lab results. Call our office if they have not been posted to your portal.  

Who should I call to discuss my bill?

            Our billing department can be reached through our dedicated line at 208-571-3779.

What should I bring for my first visit?

All new patients will need to bring in their insurance card, photo ID, and the New Patient Registration forms that can be found on our website under ‘Patient Forms’.

Will someone call to remind me about my appointment?

You will receive a confirmation text the week before. If you do not respond, the office will call you to confirm your appointment. You must confirm your appointment two days prior to your appointment or it will be cancelled.
